Cotrel-Dubousset Instrumentation on Bursting Fractures of The Thoracolumbar Junction


Abstract
The thoracolumbar junction is a mechanically and neurologically distinct region and can be limited to the area between the 11 th thoracic and 2nd lumbar vertebrae.
We analyzed 20 cases of bursting fracture in this junction which were treated by CDI at Chosun University Hospital between 1991 and 1993. The purpose of this study was to evaluate the radiologic sign. The indirect reduction. The complications.
And
the
functional results.
@ES The results were as follows:
@EN 1. According to McGrory's Criteria to evaluate the posterior vertebral angle of bursting fractures. 16 out of 20 cases(80%) were positive and the average angle degree was 106.1¡Æ.
2 Indirect reduction was achieved in 10 cases(50%) and we obtained a good indirect reduction even though canal compromise was over 50%.
3. Complications included implant failure(2 cases), pain in operation site(2cases). neurologic exacervation(1 cases), and urinary tract infection(1case).
4. According to Denis' pain and work scale. 75% were above P2W2.
This results were same as the results of patient's subjective satisfaction.
In summary we recommend doing a posterior instrumentationfirst for stability and indirect reduction, then performing a strut graft anteriorly if necessary.
